QUESTIONS ABOUT  YOUR APPOINTMENT
DO I NEED MEDICAL RECORDS?
We recommend obtaining medical records if you have access to them. The records must be from within the last 12 months with a diagnosis of a qualifiying condition. Doing this will save you the exam fee of $75.

If you do not have access to medical records from within the last 12 months and your condition is musculoskeletal or orthopedic in medical nature, Dr. Santo can preform a physical exam in office during your visit. This is an additional $75.

WHAT IF MY CONDITION DOESN'T CHANGE?
We know most chronic conditions do not change, however, the state of Arizona requires you provide current medical records within the last 12 months, every year. 

* Your qualifying condition can stay the same every year, your primary care doctor, chiropractor, or specialist will need to provide updated records stating the changes, if any, in regards to your condidtion.
WHAT IF I'M A RENEWAL WITH VCWC?
If you certified with us last year and are looking to renew your Medical Marijuana Card? Records may become outdated within a week or two of your card expiration. We ask that each patient call to schedule an appointment prior to their records being over 12 months old to stay in compliance with the state of Arizona's requirements.
ARE THERE ANY DISCOUNTS ?
If you are elegable for the SNAP/supplemental nutrition assistance program, the Arizona Department of Health Services offers a discount on their fee. Typically, the general state fee for each patient is $150; however, if your receving supplemental nutrition assistance, the state fee is discounted to $75.

* Your SNAP card must have your name printed on the front or you'll need to provide the most current approval letter DES would have sent to you in the mail.*

The patient for life voucher program is the only other discount offred/applyed to your medical card application.
DO I HAVE TO PAY THE $75 APPOINTMENT HOLD FEE?
You are required to pay a $75 hold fee to book your appointment. This is not an additional fee and it will be deducted from the total expence of your MMJ card. This fee is to ensure that patients show up on time for their scheduled appointment or communticate any changes that need to be made regarding scheduled appointments ahead of time.
PAYMENT METHODS?
We are a CASH ONLY office and require all fee's to be paid in cash at the time of your appointment.
**The only exception to this is the holding fee which is paid via paypal or credit/debit card when booking your appointment. Patients have the option to use paypal or a credit/debit card when booking online.
HOW CAN I STREAMLINE MY APPOINTMENT?
Paperwork must be filled out EVERY YEAR, regardless if you are a renewal or a new patient. The required paperwork can be printed at home and brought in. This paperwork can also be accessed and filled out in-office, ensure to give yourself enough time to do so if necessary.
Another way one can streamline the procces is by having your medical records faxed to our office before your appointment time.
Lastly, having your cash payment ready and the correct forms of identification with you will result in the fastest appointment time.
COUPONS
Are for first time patients to First time coupons are electronically attached to your patient profile. Once you receive your active medical card, you can redeem these coupons at Level Up dispensaries.
WHAT TO BRING TO YOUR APPOINTMENT
Required items to bring with you, if applicable:
1.) an Arizona Drivers license, ID card, or a U.S. passport for identification
2.) current medical card, if active
3.) qualifying medical records WITHIN the last 12 months stating your diagnosis
4.) proof of holding fee payment, printed off or be able to present email receipt
5.) valid SNAP forms, the card with your name on the front or the most recent approval awards letter
6.) your remaining balance in cash
7.) please also come picture ready, as we will be taking a picture of you to update your application.
DO I GET A TEMPORARY CARD?
The AZDHS does not issue a temporary card to use during the two weeks time period that you are waiting on your MMJ card. you will only be able to enter a dispensary once you have your phyisical MMJ card in hand.
